A steady high-pitched squeal is often the wear indicator, a small metal tab designed to make noise when pads get low. A grinding sound usually means the pad material is gone and metal is contacting the rotor, which damages the rotor and lengthens stopping distance. Grinding should be looked at right away.
Not always. It depends on remaining rotor thickness against the manufacturer's minimum spec, and on whether there's runout, scoring or heat damage. We measure rather than assume.
